Home Telemonitoring
Lutheran Home Care Services' Home Telemonitoring Service utilizes advanced technology
that allows us to collect vital signs and monitor our clients every day of the
week - from the comfort of their own homes. We have partnered with HomMed, the
nation's leading provider of home telemonitoring equipment, to bring the very
best telemonitoring service possible to our clients.
About the size of a clock-radio, the HomMed Home Telemonitoring system is small
and unobtrusive in the home. Each day a gentle, friendly voice guides the client
through the simple procedure to collect the vital signs (weight, temperature,
and blood sugar). The client is then asked up to 10 questions about his or her
condition that can be answered by pressing a yes or no button on the monitor.
The monitor automatically transmits the data via telephone connection to our office
where it is reviewed daily by our nurse. We can incorporate the data into trend
reports and provide them to the client's doctor on a regular basis. For those
with other monitoring needs, medical devices and attachments such as a spirometer,
glucose meter, an ECG device and more are available.

With this preventive and proactive approach to health care, we can detect health
problems before they escalate into a more serious condition and may help avoid
trips to the emergency room or hospital. Being monitored daily gives our clients
a sense of security and helps to foster feelings of independence.
