Lutheran Social Services is committed to making a positive impact on the lives of the people we serve. We recognize that most people prefer to live in their home and community for as long as they can do so safely. Towards that end Lutheran Home Care & Hospice and LIFE Lutheran Services deliver health care and related services to help individuals recover from illness or injury, live life to the fullest and continue to live in their homes – with their loved ones – where they want to be.
Established in 1978, Lutheran Home Care & Hospice provides skilled home health care, hospice care, non-medical in home care and telemonitoring services to individuals who reside in south central Pennsylvania and northern Maryland. Nurses, therapists, social workers, aides and other care providers bring compassion and healing, as well as emotional and spiritual support to help clients and their families cope with illness or injury. When cure of an illness is no longer a reasonable goal, hospice services provide people with an opportunity to live the last weeks and months to the fullest and to say goodbye with grace and dignity.
LIFE Lutheran Services, one of the first rural Programs of All-Inclusive Care for the Elderly (PACE), in the nation, opened its doors for participants in 2008. This innovative program helps clinically frail individuals 55 years of age and older to live in their homes and in the community by providing a full range of health care and supportive services. With health care coordinated by a team of skilled professionals and new opportunities to make friendships and connections with people, LIFE participants enjoy better quality of life and their families have renewed support and peace of mind.
Our Community Health Services programs are dedicated to providing exceptional care with compassion in the least restrictive environment possible. Our Services
Home Health Care – You want to remain independent at home and we will be there to help you! Whether you are recovering from illness or surgery or need help managing a chronic disease or condition, you can received skilled nursing, physical therapy, occupational therapy, speech therapy, personal care and/or medical social services from our caring and encouraging staff in the comfort of your home. You and your family will have peace of mind knowing that skilled professionals are caring for you.
Home Health Telemonitoring – Daily monitoring of your vital signs can identify changes in your condition that might become serious. An easy to use telehealth monitor in your home allows you to do this monitoring and automatically reports the results to a nurse who knows when those small changes need further attention. Imagine the peace of mind for you and your family knowing that a health care professional is reviewing this information every day!
In-Home Support – Keep your independence at home with the help of our caring, dependable staff. Tailored to meet your specific needs, services may include help with dressing and bathing, transportation, meal preparation, housekeeping, laundry, grocery shopping and errands. Our in-home support caregivers are employees of the agency, insured and have full background checks. Your family will rest easy knowing that you have dependable, trustworthy help to keep you safe at home.
Hospice Care – Our hospice team of professionals, paraprofessionals and volunteers are dedicated to caring for those living with a terminal illness and their families. They manage pain and symptoms, support the family, provide education on what to expect and teach care giving techniques. Our goal is to help our clients reach their goals and live their last days to the fullest.
LIFE Lutheran Services – Including many elements of the traditional health care system, LIFE Lutheran Services is a creative program designed to enhance the ability of seniors to live safely in their homes and communities with dignity and independence by providing access to the full range of preventative, primary, acute and long-term care services. It is coordinated by an interdisciplinary team, provided primarily in the LIFE Center in Chambersburg, PA and is supplemented by in-home and referral services according to participants' needs.
